Human in the loop
A person reviews or approves before the AI’s step takes effect.

Short answer. Human in the loop means a person reviews or approves an AI system’s step before it takes effect. The Model Context Protocol specification says there should always be a human in the loop with the ability to deny tool invocations. OWASP lists human approval of high-impact actions among its defences against excessive agency, and OpenAI’s guide names two triggers for human intervention: an agent exceeding a failure limit, and a high-risk action such as a payment. In an office, the AI drafts the meeting minutes and a named person confirms each action item’s owner before it is approved.

01

Where to put the person

Not every step needs a person. Put the check where a mistake would be costly or hard to undo.

StepExampleHuman check
Read and answerAnswer a policy question from office documentsShow the source passage; sample answers later
DraftA client letter or a set of minutesA named person reviews it before it leaves the building
Send or publishAn email, a filing, a shared linkApprove each item
Spend or deleteA payment, a refund, deleting a recordApproval by someone with the authority, logged

02

Approval that means something

  • Show the evidence next to the claim, such as the transcript line an action item came from, so the reviewer can check instead of trust.
  • Ask one question per decision. A screen of fifty items with one Approve all button is not a review.
  • Allow edit and reject, not only approve.
  • Record who approved what, and when.
  • Watch the rejection rate. If nobody ever rejects anything, the check may have become a formality.
  • Show the whole item. A person who sees only a summary cannot catch an error in the detail.

What the sources say

The MCP specification says that, for trust and safety, there should always be a human in the loop with the ability to deny tool invocations. Applications should show which tools are exposed, signal when one is invoked and present confirmation prompts. OWASP’s guidance on prompt injection and on excessive agency both list human approval for high-risk or privileged actions.

OpenAI’s guide treats human intervention as a safeguard that is especially useful early in a deployment, because it exposes failures and edge cases. It names two triggers: exceeding failure thresholds, and high-risk actions that are sensitive, irreversible or high-stakes.

India’s AI Governance Guidelines, summarised by the Press Information Bureau on 15 February 2026, set out a principle called People First: humans should retain meaningful control over AI systems wherever possible, supported by effective human oversight. The committee also recommends mandating human oversight and safeguards in sensitive and critical sectors. These are guidelines and recommendations, not statute. This is general information, not legal advice.

Where ANVE stands

In ANVE’s meeting-minutes maker, every action item must quote the transcript line it came from, and its owner needs to be confirmed by a person before the item is approved; each confirmation is logged. In document question answering the source passage is shown next to the answer. ANVE’s security page (/security) says an administrator must approve each cloud model, connector and agent tool. Agents built with a paid pilot, such as action follow-up or tender response, are not shipped products.

Questions people ask

Is human in the loop the same as human oversight?
They overlap. Human in the loop usually means a person approves individual steps. Human oversight is the wider idea that people can monitor, intervene and override, which is the wording India’s AI Governance Guidelines use.
Who should be the human?
Someone who understands the task and has the authority to refuse it. For meeting minutes that is a participant. For payments it is the person who would normally sign.
Does a human in the loop remove the risk?
No. It catches errors a reviewer can see. A reviewer who sees only a summary, or approves in bulk, will miss problems. Combine review with limited permissions and logs.
Does it make AI too slow to be useful?
Not if the gate sits only where mistakes are costly or hard to undo. Reading and answering questions from documents usually needs sampling rather than approval of every answer.
